Partnerships for Health
Overview
The Colorado Trust’s Partnerships for Health initiative was designed to help improve the coordination of health services at the community level. This effort, an expansion of The Trust’s Colorado Healthy People 2010 initiative, supported community health partnerships statewide, consisting of hospitals, local health departments, community-based organizations, government agencies and community members. Each partnership identified a Healthy People 2010 focus area important to its community. Through two cycles of funding, grants were awarded to help partnerships build, strengthen and sustain the infrastructure of Colorado communities by proactively addressing public health issues.

Evaluation Findings
An independent evaluation of this initiative was conducted by TriWest Group to examine grantees’ coordination of local community health services, strengthened community partnerships and improved health outcomes at the community level. The evaluation included a survey administered to representatives of each grantee’s key collaboration partners to assess the process and structure of their collaborative. Available for download are the full evaluation report, the report’s appendices outlining data collection methods, and a related 2008 collaboration survey.
