TriWest examined grantees’ coordination of local community health services, strengthened community partnerships and improved health outcomes at the community level. The Partnerships for Health Initiative Collaboration Survey was administered annually to representatives of each grantee’s key collaboration partners to assess the process and structure of their collaborative.
